Utah Children’s Health Insurance Program (CHIP) is a non-Medicaid program with benefits similar to standard insurance packages, for hospital services, physicians’ services, laboratory services, prescription drugs, mental health services, and preventive care such as routine Physicals, immunizations, vision screening, hearing screening, and basic dental services.

Services: All services are approved through Bureau of Eligibility for: hospital services, Physicians’ services, laboratory services, prescription drugs, mental health services, and preventive care such as routine physicals, immunizations, vision screening, hearing screening, and basic dental services.

Eligibility: For children 18 years and under, with family income below 200% of the federal poverty level and above Medicaid’s income threshold, not covered by employer or individual insurance plan and not eligible for Medicaid.

Fee: Minimal co-payment on some services
